Terminal Box mounting(placement of hardware)

I know the order of the star washer, flat washer, stud, star washer, then the nut (inside to outside). I'm just not sure what goes where. Are the first star washer and the flat washer between the box and the frame or are they inside the terminal box with the hexagonal portion of the stud? Or is the star washer between and the flat washer inside?
And I'm assuming the last star washer goes on the outside of the coil bracket with the nut????
